Appointment Policy

  • Arrive on time; more than 10 minutes late requires rescheduling.
  • Cancel at least 48 hours in advance to avoid a $50 fee.
  • We kindly ask that you pay your co-pay at the time of your visit. The co-pay is a fee you agreed to when signing up for your insurance. If you are unable to provide the co-pay, we may need to reschedule your appointment. Thank you for your understanding.

New Patients

  • Arrive 15 minutes early.
  • Bring: Photo ID, insurance card, payment method, and all prescription medication bottles or current med list.

Follow-up Appointments

  • It is essential that you have done your labs prior to your appointment – if you have not completed your labs prior and our clinical staff have not verified receipt of labs your appointment may be rescheduled
  • Bring: Insurance card, payment method, and all prescription medication bottles.
