What can you expect from your visit?

We have collaborated with Phreesia, a software company that allows you to complete and update your patient forms and pay your co-pay or any balance you may have from the comfort of your home prior to coming to your appointment. This will make your visit run more efficiently and on time.

New Patients:

Please complete your paperwork prior to arriving at the office

The visit for a new patient is the first step in a partnership to diagnose the issue(s) you may be dealing with related to your kidneys. This visit will start like any visit. The clinical staff member will take you back and collect your vitals. You will proceed to an exam room where they will review everything you provided with the completion of your new patient paperwork. This initial intake by the clinical staff member may take some time. The best way to keep this part efficient and productive is to have brought along your current medicines or at a minimum a current med list, and completed the forms to the best of your ability. Once they have completed the intake the provider will come in to discuss your medical history and ask any questions based on their review of your medical record.

Established Patients:

It is important to know that evaluating the function of your kidneys can only be done if you have completed your labs prior to your appointment. Without the result of the ordered labs, the doctor or nurse practitioner cannot provide the best evaluation of your kidney function. Getting your labs done is the most important step in managing your kidney care.

After checking in a member of our clinical team will come get you from the waiting room and they will take you back to get your vitals and review your present history of illness. They will ask you a series of questions. These questions help give the provider a snapshot of where you are in that moment and helps start the plan for managing your care.

Once the clinical team member has completed their portion of the visit, they will leave you for a moment before the provider comes in to review the present history of illness and to review your labs or any other tests that they had ordered from your last visit.
